An endoscopic endonasal prelacrimal recess-maxillary sinus approach for giant schwannoma of the pterygopalatine fossa in children

چکیده
Primary tumors of the pterygopalatine fossa in children are rare, and most are schwannomas. These types of tumors usually present with vague and nonspecific radiologic characteristics and it’s difficult to diagnose preoperatively. The radiologic characteristics of different kinds of tumors were discussed and analyzed. Magnetic resonance images combined with CT angiography can provide very important information to confirm tumor characteristics before operation, especially enhanced MRI. The pterygopalatine fossa is so deep that it’s challenging to reach through traditional operative approaches, and typical approaches may also cause great trauma. Endoscopic endonasal micro-invasion approaches have gradually been adopted for lesions of the pterygopalatine fossa, including those through the pterygoid process, the middle meatus, the middle meatus combined with a Caldwell-Luc incision, and the prelacrimal recess-maxillary sinus. A prelacrimal recess-maxillary sinus approach has the advantages of being short and direct, causing less trauma, offering safety and face protection, reserving the nasal structure and function, being easy to repair and review, and allowing for a second operation.

Background : Our aim in this study was to recognize the endoscopic anatomy of the Pterygopalatine fossa (PPF) and the anatomic variations of the related neurovascular structures, to define the endoscopic endonasal approach to this region. Methods: In a case series study 17 fresh adult cadavers were studied by endoscopic endonasal approach. To reach the pterygopalatine fossa endonasally, we ...
متن کاملEndoscopic endonasal approach for mass resection of the pterygopalatine fossa
OBJECTIVES Access to the pterygopalatine fossa is very difficult due to its complex anatomy. Therefore, an open approach is traditionally used, but morbidity is unavoidable. To overcome this problem, an endoscopic endonasal approach was developed as a minimally invasive procedure. The surgical aim of the present study was to evaluate the utility of the endoscopic endonasal approach for the mana...
متن کاملEndoscopic endonasal approach to the maxillary strut: anatomical review and case series.
OBJECTIVES/HYPOTHESIS The maxillary strut is the bone that separates the foramen rotundum and superior orbital fissure. Tumors involving the lateral wall of the sphenoid sinus, posterior ethmoid, or posterior maxillary sinus may invade this region. The authors detail the anatomy of the strut and present a case series that emphasizes the importance and utility of this useful landmark during an e...
متن کاملThe maxillary recess of the sphenoid sinus.
The most studied recess of the sphenoid sinus seems to be the lateral one, which is highly variable. Less attention seems to be paid to the maxillary recess, extending anterior, inferior and lateral towards the maxillary sinus. Twenty patients were referred for CT of the paranasal sinuses (axial CT), during a 3-month period. After examining the morphology of the sphenoid sinuses, the authors fo...
متن کاملEndoscopic Endonasal Transturbinate Approach to the Pterygopalatine Fossa in the Management of Juvenile Nasopharyngeal Angiofibromas
Pterygopalatine fossa (PPF) is a difficult-to-access anatomic area located behind the posterior wall of the maxillary sinus. Juvenile nasopharyngeal angiofibroma (JNA) often affects this area, and the management of feeding artery to the tumor is important in the surgery. Endoscopic endonasal approach to the PPF without endangering all other nasal structures is useful in the management of JNA. W...
